Form Submittal
The DHF form submittal process has been updated with the latest technology to enhance our customer’s ability to get information to us in an efficient and timely manner. Our forms have been modified to allow users to type directly into them then print, sign, and mail them to DHF. Please select the appropriate form by clicking on the link next to the form’s name that indicates the file size. Once the form opens in your Adobe Reader window, fill out the form by clicking in the empty fields and typing in your info. The forms now auto-fill repeated information, so you no longer have to retype data like name, address, date, horse’s name, etc.

For DHF Event Participation
If you are coming to DHF to participate in an event, you can download the DHF Event Participant, Auditor, and Horse Show Entry Forms here. If you have previously registered for a DHF event, we have your info on file, so it is not necessary to fill out all the info again, unless something has changed. Simply provide your name, event info, and check the box indicating you have previously registered for a DHF event.

For DHF Services
Every horse on DHF property needs a current negative Coggins certificate (within the 12 months).

Every person needs to sign a DHF Liability Release.

If you and your horse are boarding or training, you need the Incoming Horse Package.
